I used quiska  Font free Here

Preset shape of your choice

FTU Black and gold kit from Dreamworld Here

Mask of your choice 

 

Filters:

Eye candy gradient glow thin

Xero- bad dream

Drop Shadow 2- 2 -35 -5

Lets Start:

Open a image 851 x 315 pixels

Copy and paste paper 9

copy and paste paper 10

Change blend mode to overlay

Copy and paste element 48 resize 70% position to the left

Change blend mode to hard light

Copy and paste element 45 position to the left bottom resize 50%

Copy and paste element 25 resize 50% position to the left.

Copy and paste element 19 position in the right side of the banner

Copy and paste element 13 position in the right side near to the bottom center

Copy and paste element 10 position to the bottom

change blend mode to Luminance(legacy)

Ad other elements of your choice

 Add your close up tube and position to the right

Duplicate

Adjust blur Gaussian blur by 10

Change blend mode to overlay

Crop your tag

Click in your top layer

copy and paste paper  2

Crop your tag 

Selections all selections modify contract by 12 delete

Copy and paste paper 8

Crop your tag

Selections all selections modify contract by 6

delete

Copy and paste paper 2 again

crop your tag

Selection all selection modify contract by 2

delete

merge down this 3 layers

Apply eye candy gradient glow- thin   wit this settings:

Merge visible

Apply filter Xero-Bad dream with this settings:

 Add your copyright

Ad your name.

EFFECTO FOR THE NAME

You need :

Eye candy 5 impact - bevel

Photoeffex-scanlines

Eye candy gradient glow 4000

For my name i used Quiska font

Foreground null Background black

write your name

Apply effects- 3D effects  inner bevel with this setthings:

now apply effects eye candy impact bevel with this settings:

Now apply photoeffex-scanlines with this settings:

apply inner bevel again

apply scanlines again

Drop shadow

now go to eye candy 4000

gradient glow thin with this settings:

But change the color to a golden color

this is my result:

Go to your tag close off the tube, copy merged, paste, resize to 60%, adjust 
how you like,

new raster layer

copy and paste paper 2

crop

 Selections all selections modify contract by 6 delete
